• County Clerk Birth Certificate
• Proof of income for the past 30 days (e.g. two consecutive pay stubs, TANF statement, Social Security statement, child support, unemployment compensation)

Enrollment Requirements 

• Proof of Residency
• Medical Card 
• Physical
• Up-To-Date Immunizations 
• Lead Test With Results
• TB Test with Results  
• Hemoglobin Test
• Allergy Medical Statement if Applicable
• Asthma Medical Statement if Applicable
• Medication Authorization Form if Applicable
